• Use the online application form to apply and set up your direct debit online. You will need to have your account information (as shown on your cheque book and bank statement) to hand.

Or, the application form for NATFHE Membership and Associateship can be downloaded in two different formats:

• A Word document version that can be filled in on screen, then printed and signed; or printed blank and filled in manually.
• A large print Word document version that can be filled in on screen, then printed and signed; or printed blank and filled in manually.
